Case Manager

Position Overview: Assist with the distribution of township welfare funds through General Assistance and Emergency Assistance programs. Administer auxiliary services offered by the department to township residents. Represent the township at networking and social events. Reports to the Director of Social Services. Essential Duties & Responsibilities: Casework Determine client eligibility for various township programs through an interview process and application of knowledge of state guidelines  Establish ongoing eligibility for existing clients Disburse approved Township funds to qualifying clients Maintain current records, in electronic/paper format Attend semiannual General Assistance Training Institute (GATI) and other trainings as assigned Provide information and referral to other community resources Advocate for clients with outside agencies Assist clients with completion of applications for, but not limited to: Access to Care Mobile Dental Clinic Salvation Army LIHEAP Information and Referral  Screen clients for eligibility for various non-township programs using knowledge of current guidelines Provide additional referrals to other townships or community resources Other Expectations and Requirements: Assist with screening and administering Holiday Programs such as Hope for the Holidays, Thanksgiving Baskets, etc. Form and maintain an understanding of Visual GA software Write and maintain professional case notes in Visual GA Verify and update Additional Assistance Files on Visual GA for each client as changes occur (Address, Contact information, Household members, etc.) Scan/Upload all documents pertinent to each case and upload to Visual GA Complete training and assessment to be recertified as an Energy Service Technician each fiscal year to continue processing all CEDA program applications (LIHEAP, Furnace, and Weatherization) Request all documents or information pertinent to the case from clients and review completed applications Process disbursement orders for General Assistance/ Emergency Assistance cases Assist with covering the front desk lunch break hours as needed Schedule new and existing Food Pantry appointments Renew and issue new Food Pantry ID cards for clients as needed Check voicemails throughout the day (Respond to client inquiries within 1 business day) Attend trainings virtually or in person to stay up to date on current programs as well as learn new ones Additional Responsibilities: Assist, as needed, with seasonal programs, which include application, coordination, and distribution Attend community events, networking events, and trainings on behalf of the township Clerical assistance as needed Attend occasional meetings outside of business hours Other duties as assigned by the Director of Social Services Credentials and Experience: Bachelor’s degree preferred in a social services-related field Desire to work with those in need Willingness to be flexible, learn new things, and ask questions Ability to work in a fast-paced office Experience with Visual GA program or bilingual desired, but not necessary Applicant must hold a valid Illinois driver’s license Work Environment: Work Location: On-site Elk Grove Township maintains a professional work environment where employees are expected to uphold a high standard of conduct, dress, and professionalism in their interactions with the public and colleagues. Bus Driver

Rotates driving one of four (4) Elk Grove Township buses (passenger units equipped with handicap lifts) within Township boundaries. Hours of operation for the Senior and Disabled Transportation Department are 9:00 a.m.-4:00 p.m. This is a full-time position with the opportunity to work 40 hrs/week by assisting the Township Food Pantry with Food Rescue. Reports to the Transportation Manager and works closely with the transportation team that books reservations and coordinates the daily transportation process. Day-to-Day Operations: Review itinerary each day prior to the start of route by logging into tablet. Transport all senior citizens & disabled passengers requesting trips to dialysis, doctor appointments, shopping, senior centers, special trips, etc. Assist in coordinating passenger pick-up times and return trips home Keep Administrative personnel informed of any concerns with all passengers’ health and their ability to travel (i.e. – ambulatory, confused, etc.) Assist with Food Pantry pickups 3 days a week for Food Rescue. Safety Training: Complete training for First Aid and CPR/Defibrillator use Trained on passenger safety: handicap lift on all Township vehicles, including use of restraints for wheelchair hook-up(s) Maintenance: Perform safety checks (oil levels, conditions of tires, brake lights, mirrors, directionals, emergency brake) on the bus each morning before beginning route to ensure all are in good working order and communicate any safety/maintenance issues Fueling buses and recording usage amounts for financial record-keeping Responsible for periodic bus cleaning throughout the year Skills: Excellent communication skills Able to interact with diverse groups of people Capable of following established rules & guidelines of transportation program Requirements: Hold a CDL with a ‘P’ (passenger) endorsement and keep CDL current Temperament to work with seniors and disabled bus passengers Other: Duties that may be assigned, such as assisting with creating daily bus routes, occasional office work, distributing approved Township information to bus riders, and being versed in other Township assistance programs Elk Grove Township does not discriminate on the basis of race, creed, color, ancestry, national origin, age, sex, or handicap and complies with the provisions of the Equal Employment Opportunity Act.
