Qualifications:

Individual must possess Supportive Living or Skilled Nursing background. Minimum of three (3) years’ experience in Compliance and Medicaid for long term care facility and/or other agencies. Individual should have prior experience serving as a liaison between families, healthcare professionals and administration. Knowledgeable in LIHTC, HUD, fair housing, HIPPA, and other programs.

Responsibilities:

  • Ensure that move-in paperwork is complete and that all necessary forms are completed and distributed to the DCR.
  • Assist family and residents with move-in process.
  • Work with Compliance and Medicaid to make sure all necessary paperwork is submitted with the application. Follow up with family to make sure that all paperwork/documents are gathered correctly
  • Act as the primary liaison to the family for Sales, Compliance and Medicaid and utilize customer service skills to communicate additional information that is needed for the application to be approved.
  • Complete criminal background check, order DON screening, gather medical information necessary for move-in.
  • Submit Medicaid applications, complete Medicaid redeterminations
  • Meet with families and/or prospective residents to help facilitate the application process.
  • Enter new resident required information into Yardi upon move-in.
  • Act as a back-up to the Lifestyle Specialist when needed. Respond to all new inquiries immediately (phone-ins and walk-ins) or before the end of the day.
  • Skillfully interview prospects or their representatives to identify how Grand Lifestyles may meet their needs.
  • Effectively communicate and explain the application processing policy, and the current resident referral policy the types of floor plans, the basic features of each type and the availability of units within the community;
  • Monitor Vacant Units onsite vs System tracking. Partnering with ED/Maintenance Manager on unit readiness and compliance the financial criteria required to move in, including conducting a pre-qualification on potential residents to ensure they are appropriate and meet the financial guidelines of the community.
  • Provide prospective residents with a list of documents and all state-mandated information required.
  • Coordinate move-in dates and apartment assignments for new residents in accordance with open apartments and vacant units.
  • Communicate with and involve appropriate community team members in the decision-making process required to determine residency.
  • ADDITIONAL DUTIES:
    • Maintain regular contact with prospective residents and their families to validate their interest and move them towards an approved application and record updates to customer information in YARDI.
    • Work with the Executive Director to communicate to team members information on sales issues impacting other departments.
    • Perform related duties as assigned or as the situation dictates

Requirements:

  • Must have strong customer service, communication, and organization skills.
  • Must demonstrate empathy and compassion to connect with customers.
  • Must maintain confidentiality, especially with respect to resident health and financial information.
  • Familiarity with computer-based software and applications for capturing sales data and word processing, in addition to knowledge of Microsoft Word, Excel, email and Internet functions.
  • Understanding and effective implementation of all appropriate fair housing regulations.
  • Must prepare all resident initial and recertification lease packages for signing, including addendums, HIPPA authorizations, Tenant Income Certification, and extraneous documents in an organized fashion
  • The ability to read, write and follow oral and written direction in the English language at a level adequate to perform the job duties.
  • The Ability to effectively communicate, verbally and in writing, with potential customers, families, and the public.
  • Ability to work with the geriatric population.
  • Must be available to work evenings, weekends, and holidays when necessary.
  • Must be able to assist in the evacuation of residents.
  • Must have experience in processing income verifications, mortgage processing, and/or tax credit regulations.
  • Must be able to inform of federal, state, and local policy, and community specific regulatory requirements such as LIHTC, HUD, fair housing, HIPPA, and other programs.
